Vascular Risk Assessment
(NHS Health Check)
This assessment is offered to people aged 40-75 years with no existing chronic disease. It focuses on factors including smoking, healthy eating, alcohol use, physical activity and emotional wellbeing.
The information gathered helps people plan for lifestyle change, giving ideas, information and support. We aim to invite all our patients that fall into this category within the next 2-3 years but please feel free to contact the surgery to arrange an appointment to see our Healthcare Assistant.
